How to Obtain a PIVA Extract / Proof of Address

If you need a PIVA extract (Proof of Address), please follow the steps below:

1. Who Can Request a PIVA Extract?
  • You must request the extract in person.
  • Each person must request their own extract – even if you are married, your spouse cannot request it for you.
  • You must bring a valid ID card or passport.
2. Where to Request It?
  • You can only request the extract at the Census Office window.
3. Fees & Payment
  • The cost for a PIVA extract (Proof of Address) is $9 USD.
  • Payment must be made at the Receiver’s Office when collecting the document.
4. Requesting on Behalf of Someone Else?

If you need to collect the extract for someone else, you must present:

  • A completed authorization form (signed by the authorizing person).
    • Authorization form can be downloaded, see below.
  • A copy of the valid passport of the authorizing person.
  • Your own valid ID card or passport.
